Claxton-Hepburn Medical Center Celebrates National Health Center Week

Claxton-Hepburn Medical Center is joining in the National Health Center Week campaign that runs August 13 to 19, with goal of raising awareness about our seven area health centers, located in Canton, Ogdensburg, Madrid, Waddington, Lisbon, Heuvelton, and Hammond, that provide primary care to adults and children in their surrounding communities.  One convenient number, 315-713-6333, can be called for scheduling services at all of our health centers. 

According to the National Association of Community Health Centers, health centers are consumer-driven and patient-centered care sites that serve as a comprehensive and cost effective primary care option for America’s most underserved communities.  Health centers serve everyone regardless of ability to pay or insurance status.  They increase access to health care and provide integrated care services based on the unique needs of the communities they serve.  Health centers deliver value and have a significant impact on Americans healthcare system.

Claxton-Hepburn’s health centers serve as a Patient Centered Medical Home (PCMH) model that places you the patient, at the center of the healthcare system, and the medical home team acts as the core of your healthcare, managing your care and services for you.    As a PCMH, we provide primary care that is accessible, continuous, comprehensive, family-centered, coordinated, and compassionate to all patients, regardless of their ability to pay.

Our services at the Health Centers include primary are for adults, adolescents and children, imaging and lab services, orthopedics, care coordination, and behavioral health support.  We also welcome DOT and school physicals. 


The health and wellness of our patients is our number one priority.  Providing the best possible care to every patient is our primary goal.  By working together as a team, we can meet this goal.  Your team may include your doctor, health coaches, care managers, social workers, and other professionals based on what you need. 

In addition to our seven area health centers, Claxton-Hepburn provides after-hours care at our Convenient Care clinic located in Ogdensburg.  Health problems don't stick to a set schedule. You're just as likely to deal with new symptoms on the weekend as you are during the workday. We can help. In our after-hours clinic, we offer walk-in appointments for urgent care issues, and we can assist with some forms of routine medical care too.  Due to increased demands, we recommend you call Convenient Care ahead of time at 315-393-1715.
